Reconstruction of Soft Tissue Defects Around Ankle with Perforator Propeller Flap

Objectives
Reconstruction of soft tissue defects around the ankle is an ongoing challenge for reconstructive surgeons. This study aimed to evaluate the outcomes and complications in patients with soft tissue defects around the ankle who underwent reconstruction using a perforator propeller flap.
Methods
The medical records of 17 patients who underwent perforator propeller flap procedures for soft tissue reconstruction around the ankle at our hospital between January 2021 and January 2025 were reviewed. Defect etiology, flap dimensions, arc of rotation (degree of rotation), flap survival and complications were recorded.
Results
The patients’ average age was 41 years, the mean defect size was 5.1 × 7.6 cm, and the mean flap size was 5.8 × 14.1 cm. The mean flap rotation was 152°. The donor site was closed primarily in three patients, and split-thickness skin grafts were applied in 14. Partial necrosis at the flap tip was observed in three patients, and in one patient, partial epidermolysis was observed.
Conclusion
Small and medium-sized soft tissue defects around the ankle can be successfully reconstructed using perforator propeller flaps, which provide tissue similar to the native local tissue and minimal donor site morbidity.
